- Under the direction The Pharmacy Team Coordinator, Population Health, provides ambulatory pharmaceutical care services in collaboration with other health professionals.
This may include performing various patient-focused clinical, educational, and informative functions in an ambulatory care setting as an interdisciplinary team member in support of patients, practitioners, and ancillary staff.
Collaborates in the development and/or implementation of disease-state management pathways, medication initiatives and other formulary management activities. - DLD - Doctorate Level Degree in Pharmacy
- Minimum 3 years experience as a Pharmacist in a hospital, clinic setting or long term care
- Completion of ASHP-accredited residency may be substituted for required experience.

This is a Full Time position with day shift hours and great benefits
Position Status:
Full-time, Day Shift
Standard Working Hours: 8:00AM to 5:00PM (ET). Hybrid work schedule. Will travel 2 x a week to the office in Charlottesville, VA.
Job Responsibilities:
Activities include conducting patient outreach to complete comprehensive medication reviews and medication reconciliation, medication related quality gap closure, activities supporting transitions of care, provider consultation and other activities related to medication-related outcomes for our attributed populations, including SQCN and SACO (Commercial, Medicare, Medicare- Advantage, and Medicaid).
