Provide hands-on assistance with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
Assist with meal preparation, feeding, and ensuring adequate hydration
Administer basic health monitoring and report any changes to the community director
Offer companionship, comfort, and emotional support to residents
Lead or assist with engaging activities and social interactions to promote resident well-being
Perform light housekeeping and laundry tasks to maintain a clean, comfortable environment
Help with transporting or transferring residents safely as needed
Participate in care team meetings and contribute to personalized care planning
Support a safe and nurturing living environment, attentive to residents' physical and emotional needs
Uphold Fox Trail's values of respect, compassion, and person-centered care
Experience & Qualifications:
Current New Jersey Certified Home Health Aide (CHHA) license required
Certified Dementia Practitioner (CDP) preferred
Prior experience in memory care, senior care, or assisted living is a plus
Must be at least 18 years old and eligible to work in the U.S.
Willing and able to lift over 75 pounds for tasks such as transferring and supporting residents safely
Required Skills:
A heart for service and compassion for individuals living with Alzheimer's and dementia
Excellent communication and interpersonal skills
Dependability, patience, and a strong work ethic
Ability to work independently and as part of a collaborative care team
Physical stamina and willingness to assist with mobility and physical care
Dedication to upholding the dignity, comfort, and well-being of every resident
This position is available for Full Time/Part Time/ Per Diem/ ALL Shifts
