Job Summary:

We are seeking an experienced RN Care Manager or Social Worker to join our high-performing Case Management team. As a key member of this team, you will play a critical role in driving positive patient outcomes by leading complex care coordination, connecting patients with essential resources, and supporting safe and successful transitions of care.

Responsibilities:

• Patient Assessment: Conduct thorough assessments of patients' physical, emotional, and social needs.
• Care Planning: Develop individualized care plans that address patients' unique needs and circumstances.
• Care Coordination: Collaborate with healthcare teams to ensure seamless transitions of care and coordinate necessary services such as home health aide services or medical equipment rentals.
