    Description

    Job Title: Case Manager II at Telecare

    Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders, helping them to live as independently as possible in their chosen community.

    Shifts Available:

    • Full Time; AM shifts from morning to afternoon with availability for emergency calls; Monday - Friday

    Expected starting wage range is competitive. Telecare offers geographic differentials to its pay ranges based on the location. Starting pay is commensurate with relevant experience above the minimum requirements.

    Requirements:

    • High School Graduation or equivalent with two years of direct services in Mental Health or community services (or equivalent qualifications)
    • Valid driver's license and personal vehicle insurance
    • Willingness to use the company vehicle for client transportation
    • Preference for bilingual Spanish
