    Winona Health is a registered Community Health Worker Apprenticeship site through the Minnesota Department of Labor and Industry. This may be an ideal opportunity for those interested in a Community Health Worker Training Program (CHWTP) Apprenticeship. CHWTP Apprenticeship participants maybe eligible to receive an additional short-term stipend in addition to wages.


    Position Overview:


    The Community Health Worker employs community-based care coordination strategies outlined in the Pathways Community HUB model to improve health outcomes for targeted populations.

    Through community-based care coordination this position will provide support to at-risk clients as they navigate the health care and social service systems in the Winona area.

    This position aims to increase access to support services, improve the quality and cultural competence of their care, and increase the health knowledge and self-management skills of clients served.

    The Community Health Worker has a deep knowledge and understanding of the Winona-area community, and holds lived experience that uniquely equips them to bridge barriers related to culture, race, language, socioeconomic status or other factors.

    This position requires extensive travelling throughout the community working with varied populations, oftentimes in client homes.

    This position works in partnership with the Live Well Winona (LWW) Winona Community HUB and works closely with Winona Health staff, area social service organizations and other partners.*Position may be partially or fully funded by grant(s).


    Essential Duties & Responsibilities:


    The Community Health Worker will serve as a critical linkage between health and social services agencies by providing compassionate and professional outreach to at-risk clients identified at Winona Health or partner agencies.

    The Community Health Worker will follow-up with clients in their homes or other community-based locations to conduct in-depth evaluation of their social, medical, employment, housing, financial or other social needs.

    The Community Health Worker will apply their shared life experience and training to assist clients with education, advocacy, care coordination, coaching and navigation in order to ensure these needs are systematically addressed.

    The Community Health Worker provides communication support and acts as a client advocate on issues of access and use of medical, social, or other services.

    As such, the Community Health Worker may facilitate transportation for clients to access healthcare or social service appointments, may attend appointments with clients, facilitate paperwork completion, ensure instructions are understood, and that progress towards self-sufficiency is being achieved.

    The Community Health Worker ensures the privacy and security of PHI (Protected Health Information) as outlined in Winona Health's policies and procedures relating to HIPAA compliance.

    Client interactions are documented in a timely and thorough manner using a HIPPA-compliant electronic data collection platform.
    The Community Health Worker will attend meetings and trainings as needed.
    Overnight travel to conferences or community health worker trainings may be required on occasion.
    The Community Health Worker may be required to distribute and discuss health promotion information to community groups as needed.


    Essential Skills and Experience:
    High school graduate or equivalent; some college preferred. Preference for Community Health Worker (CHW) certificate completion.
    Employment contingent upon achievement of CHW certificate within two years of hire. The certificate program is at the discretion of Primary Care leadership.

    Upon hire and completion of Community Health Worker certification be able to demonstrate an understanding of Community Health Worker (CHW) health care delivery system and the Pathways Community HUB model.

    Minimum one-year experience volunteering or working in human services, healthcare, community outreach, health education, or related direct service.
    Demonstrated understanding of various healthy coping skills.
    Intermediate computer and phone skills. Ability to use e-mail, and navigate the internet, and perform data entry in a timely manner.
    Ability to communicate effectively orally and in writing.
    Must have a passion for working in the community to address issues of inequity.

    Strong social and listening skills are desired, including the ability to build trusting relationships and work effectively with socially and medically complex clients in a variety of non-traditional settings.

    Empathy, patience, and persistence are critical skills.
    Ability to communicate collaboratively in a team while managing multiple priorities is required.
    Must demonstrate skills in creative problem-solving and attention to detail.
    Must utilize effective time management skills, and exercise sound judgement while keeping professional boundaries.
    Ability and willingness to work hours outside a standard work week when necessary.

    The Community Health Worker must have and maintain valid driver's license, valid auto insurance, good driving record and access to reliable personal transportationPhysical Demands:
    Light WorkWork activities that require lifting up to 20# rarely, 15# occasionally and 10# frequently. Push/pull 20# of frequent basis and 40# on rare basis. Light work generally exceeds the physical demand requirements for those for sedentary work.

    The work activities may include working in bending or stooping positions on occasional basis and/or rarely squatting or kneeling Physical Requirements:

    May lift and carry supplies that weigh between 10-15# and max of 20#May push/pull carts on frequent basis with force of 20# (included are laundry carts, office supply carts (Rubbermaid carts=12-15#), vacuum cleaner, meal carts)May occasionally push/pull patients/residents in wheelchair for short distance of less than 25 feet.

    May assume reaching ranges frequently between vertical heights of 20-36" in either sitting or standing position to complete work activities.

    Reaching to retrieve items from storage may require occasional reaching at vertical heights of 18-20" and 40-72".May assume frequent reaching to the front and side and occasional reaching above shoulder height while working in either sitting or standing positions.

    May require light to moderate grip or pinch force to complete work activities.


    Work Environment:


    May include office time for documentation, but the work environment is largely inside patient homes, or community-based locations including social service organization settings.

    This position works interdependently with patients, other community organizations, and WH social workers. Multi-tasking is standard and frequent interruptions may occur throughout the day.


    Required Work Schedule:
    Primarily Monday through Friday daytime with flexibility to meet the needs of clients outside of a standard work week
