    Responsibilities

    Provide social services to patients, families and the community as requested or deemed appropriate.


    • Understand and implement social work principles and code of ethics.
    • Conduct discharge planning, crisis counseling, community resource information and education.
    • Assess the patients' services to determine whether they are successful.
    • Work effectively within a multidisciplinary team environment.
    Requirements


    Licensure / Certification / Registration:

    • Social Worker credentialed from the Texas Department of State Health Services required.

    Education:

    • High School diploma equivalency with 2 years of cumulative experience OR Associate's
    degree/Technical degree with 1 year of experience OR 5 years of applicable cumulative job specific experience required.

    Additional Preferences


    • 5 years of social work experience required.
