    Role: Physical Therapist

    Project Type: Contract Assignment

    Pay Range: $62–64/hour

    Location: Cortlandt Manor, NY 10567

    Shift Schedule: 8:00 am - 8:30 pm (includes a 30 min break)

    Required and Preffered Qualifications:

    • Active New York State Physical Therapist (NYS PT) License (must have)
    • Outpatient experience (preferred)
    • EPIC experience (highly preferred)
    • CPR Certification (preferred)

    Project Description:

    Provided outpatient physical therapy services by evaluating patients, developing individualized treatment plans, and administering therapy interventions to improve mobility, restore function, and prevent disability following injury or physical limitations.

    Key Responsibilities:

    • Performed patient evaluations and assessments to determine therapy needs
    • Developed and implemented individualized treatment plans based on patient condition and goals
    • Directed and administered therapeutic treatments and exercises to improve functional outcomes
    • Monitored patient progress and modified treatment plans as needed
    • Educated patients on proper movement techniques and injury prevention strategies
    • Maintained accurate documentation of evaluations, treatments, and patient progress
    • Collaborated with healthcare providers to ensure continuity of care
    • Ensured compliance with facility policies and professional standards of practice
