We are seeking a licensed social worker to provide emotional support and resource coordination to patients and families navigating end-of-life care. As a vital member of our interdisciplinary team, you will be the steady hand and kind voice when it's needed most.- Must be a Pennsylvania resident
- Bachelor's or Master's degree in Social Work (BSW/MSW)
- Hospice experience is a strong plus
