Community health centers seeking Family Practice and Internist providers for two of their network of 30 accredited, Join commission-Certified clinics located along the pastoral California Central Coast; a non-profit organization, they are dedicated to providing a patient-centered medical home for the ethnically diverse, medically underserved populations of San Luis Obispo and Northern Santa Barbara Counties.
Under their umbrella, their multi-specialty group practices place providers within a capable team of medical and support staff, offering physicians and providers a comprehensive network of support; productivity will also benefit from state-of-the-art EHR and EPM;
Service population is primary care, all life cycles, adults 75% (chronic disease, health maintenance, pain management, mental health), Pediatrics – 5-10% (well babychild/chronic disease, health main and Women's Health 15% (Health Main, Pap smears, Family Planning Services.
