    • The shift for this position is 40 hrs.Mon - Fri 07:30a - 3:30p
    • The pay rate range for this position is $22.50 per hour
    • This position is Interim with the possibility of full time hours

    Qualifications:
    • Associates Degree in Human Service or Equivalent Credits required. Bachelors degree preferred
    • Minimum two years experience in ID/DD or related human services background.
    • Knowledge of OPWDD Waiver Services preferred
    • Excellent communication and written skills
    • Valid NYS driver's license
    • Supervisory Experience Preferred
    • Proficiency in computer skills required
    Key Responsibilities include but are not limited to:
    • Oversee and supervise the day-to-day operations of the Day Habilitation site as it pertains to program participants, staff and program site.
    • Provide direct supervision to the Assistant Supervisor and Day Habilitation Support Staff to include consistent support, coaching and training, retention strategies, monthly staff meeting and 1:1 supervisory support meetings as needed.
    • Provide ongoing protective oversight to individuals in the Day Habilitation program.
    • Provide assistance and vocational instruction to each individual in basic activities of Daily Habilitation Services to include travel training.
    • Provide training and support for eachprogram participant in accordance with the Day Habilitation Plan and the PCP.
    • Develop, implement and review Day Habilitation plans including the POM.
    • Ensure adequate staffing at all times.
    • Drive agency vans and demonstrate safe driving practices.
    • Assist with following the vehicle maintenance schedule. Follow up with staff on any vehicle related issues according to agency policy and guidelines. Monitoring and updating the vehicle trip logs, first aid kits, parking permits.
    • Monitor Fleetmatics and Motorist Safety Reports.
    • Ensure quarterly maintenance of fire extinguishers (North Shore). Order water/office supplies as needed.
    • Conduct monthly fire evacuation drills and record in fire safety log.
    • Attend all in-service training, and staff meetings.
    • Ensure regulatory compliance by reviewing/ auditing pertinent documentation for QA and fiscal audits to include review of ISP checklists and van log/daily activity sheets. Ensure staff training as per regulations.
    • Ensure incident reporting procedures as per regulations and agency policies.
    • Ensure for accurate and timely submission of monthly billing. Audit Medisked monthly notes.
    • Maintain effective relationships with volunteer sites, family members, IRA staff, MSC providers and outside recreational based activity centers.
    • Collaborate with Customized employment team to ensure individuals are provided with employment opportunities.
    • Conduct monthly "in the field" support checks for staff in the community.
