    Northwestern Healthcare Center , a member of the

    CommuniCare Family of Companies , is currently recruiting for an

    Admissions Sales Director to join our team.

    PURPOSE/BELIEF STATEMENT:


    The position of Admissions Coordinator is responsible for the coordination of admissions into the center and for assuring that all required paperwork is completed prior to day of admission.

    While focusing on supervising the delivery of quality care, the position must also manage the resources within their control and assist others in managing resources.

    JOB DUTIES & RESPONSIBILITIES
    Give tours of the center.
    Handle all daily admission calls to the center.
    Follow up on inquiries made after hours.
    Follow up on all referrals daily to check current status.
    Follow up on admissions concerns from residents, family, and staff.
    Complete referral tracking worksheets.
    Take pictures for possible admission.
    Maintain a waiting list.
    Review all fax referrals and send out to Review Team.
    Contact families and social workers when a bed becomes available.
    Send out bed availability sheets to referral sources.
    Coordinate hospital/home visits.
    Ascertain financial status (Private, Medicaid, Medicare).
    Meet the Director of Nursing to obtain approval for admission. Provide Executive Director with injuries that are rejected.
    If Medicaid – assure approval prior to admission
    If Medicare – notify Review Team for approval (if applicable).
    If Worker's Compensation or insurance claim – obtain approval prior to admission.
    Notify staff of new admissions via am census meeting and on communication forms.
    Perform all admissions processes.
    Perform all census procedures
    Perform all discharge procedures.

    Maintain up-to-date knowledge of the financial aspects of nursing operations and provide accurate documentation of all services in order to assure accurate reimbursement from all payor sources.

    Manage the resources available in the role of admissions coordinator while ensuring that needed resources are available and that quality care is delivered to all residents.

    Maintain and work within established departmental, center, and corporate policies and procedures, objectives, quality improvement program, and safety, environmental, and infection control standards.

    Promptly report any suspected resident abuse, neglect or misappropriation to the DON.

    Participate in and provide scheduled training, educational classes, and orientation programs to maintain licensure of self and certification of team members and enhance quality of care.

    Perform other related activities as assigned or requested.
    Act as back up to marketing director.
    QUALIFICATIONS, KNOWLEDGE/SKILLS & ABILITIES
    3-5 years of professional experience, preferably in a long term care environment.
    Prior supervisory experience and management training.
    Must be knowledgeable of Medicare and Medicaid requirements and procedures, as well as laws, regulations, and
    guidelines that pertain to long-term care.
    Must have the ability to make independent decisions when circumstances warrant such action.
    Must be able to deal tactfully with team members, other staff, residents, family members, visitors, government
    agencies, and the general public.
    Must be able to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures, etc. that are necessary for providing quality care.

    Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.

    Must possess basic computer literacy skills
