GENERAL SUMMARY
Works under the supervision of the Nurse Manager.

The Home Care Intake Nurse LPN's primary job function is to receive and process referrals and coordinate Home Care admissions.

Serves as liaison for Home Care services with all referral sources. Serves as resource to physicians, other referral sources and Home Care staff on reimbursement issues. Verifies reimbursement rates with payer sources and obtains prior authorizations and reauthorizations as needed. Completes ICD coding and assists with QAPI programs. Performs other duties as assigned.


QUALIFICATIONS

Education:

• High School Diploma or successful completion of an equivalent High School Exam Required
• Graduate from an accredited school of practical nursing required
• HCS-D or other ICD Coding Certification preferred

Licensure:

• Licensed to practice as an LPN in Ohio required
• Licensed to practice as an LPN in Kentucky to be obtained after hire required

Experience:

• One year nursing experience preferred
• Prior home care experience preferred
• Good computer data entry skills required
• Will be required to learn ICD-10 Coding

JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS
The following is a summary of the major job duties of this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time.

• Receives and processes referrals for all Home Care services.
• Verifies Home Health coverage and/or obtains prior authorization from alternate payer sources. Communicates this information to appropriate personnel.
• Tracks and monitors visit authorization- submits timely request for reauthorization when needed and communicates this information to appropriate personnel.
• Documents referral information in the patient record. Initiates referrals for skilled home care services into Delta automated documentation software.
• Assists in discharge planning from hospitals and institutional settings to insure continuity of care for the patient.
• Serves as a resource for resolution of billing issues and problems.
• Assigns ICD Codes to primary and secondary diagnoses and surgical procedures.
• Assigns and coordinates all admission visits.
