    Description of Primary Responsibilities
    1. Plan and coordinate all scheduling activities related to transportation for FHCN PACE participants. Work with Interdisciplinary Team, participants, and family members to ensure pick-up and delivery of participants are within acceptable time frames.
    2. Supervise the activities of the Drivers daily. Receive reports regarding participant status and communicate concerns to the Interdisciplinary Team.
    3. Ensure drive times and participants' time in vehicles during transit are within regulatory requirements.
    4. Oversee competency requirements and training of Drivers, including body mechanics and use of mechanical assistance to transport participants.
    5. Occasionally serve as Driver depending on staffing needs and be up to date with Driver competencies and licensure/insurance requirements.
    6. Coordinate all outside service provider transportation arrangements, including proper vehicle maintenance and condition.
    7. Assure all necessary program records are maintained according to policy.
    8. Participate as a member of the Interdisciplinary Team: attend staff meetings, take part in participant care planning, and communicate any reported changes in participant condition to team members.
    9. Will serve as backup Driver. Assists in the coordination of 24-hour care delivery with assistance from FHCN's Navigation Center. Operate a medical van to assist participants in safe transportation to and from all appointments.
    10. Other projects and duties as assigned.
    Description of Primary Attributes

    Professional & Technical Knowledge:
    1. Must possess a high school diploma or General Educational Development (GED) certificate and be at least 21 years of age.
    2. Minimum 1 year experience transporting individuals with special mobility needs.
    3. Valid Driver's license and an acceptable driving record required.
    4. Upon employment, defensive driving course, first aid, and CPR certifications are required to be kept up to date.
    5. Interact in a friendly and professional manner with participants and their families.
    6. Communicate effectively and openly with other Interdisciplinary Team members on patient status and needs.
    7. Only act within the scope of his/her authority to practice.
    8. Meet a standardized set of competencies for the specific position description established by FHCN PACE and approved by regulatory agencies before working independently.
    9. Pass a criminal background which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug or alcohol abuse.
    Technical Skills:
    1. Ability to prepare basic correspondence and simple reports in Microsoft Word.
    2. Ability to use Microsoft Excel to create tables and simple displays of information.
    3. Ability to create basic presentations in Microsoft PowerPoint.
    4. Proper body mechanics (lifting and transfer techniques) for assisting seniors and persons with mobility limitations.
    5. Managing special needs populations, including individuals with dementia.
    6. Latest technology for proper dispatch of transportation services and communication protocols.
    Licenses & Certifications:
    1. Currently certified in cardiopulmonary resuscitation (CPR), if CPR is not current, will be granted 30 days from date of hire to schedule for next available CPR certification class provided by FHCN.
    2. Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

    Physical Demands:

    The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.     • Physical demands: Employee may be required to conduct home visits to private homes of FHCN PACE participants for transportation pick-ups. On occasion, when fulfilling Driver duties, Employee will be assisting elderly patients during transit to and from the Adult Day Health Center. When acting as a Driver, Employee will have significant hands-on care of participants and the usual and customary methods of performing the job's functions require the following physical demands to assist participants with ambulation: significant lifting, carrying, pushing and/or pulling, stooping, grasping, kneeling, crouching. There will be repetitive motion of the wrists and hands. Employees must be able to stand, bend, and walk for extended periods of time. As a Driver, an Employee must be able to sit and remain alert for extended periods of time while operating a vehicle. Employees are required to use arms and legs during transit escort. Employees may also, on occasion, have to assist with lifting participants, and help them turn, walk or stand. Ability to lift/move up to 75 lbs occasionally, and up to 50 lbs frequently.
    • Work Environment: Employee must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the FHCN PACE members' care. On occasion, the Employee will spend time in a vehicle transporting participants. The primary work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled office temperatures.
