    Are you looking to make a difference in the lives of children and their families? We are hiring a

    Care Coordinator


    for our Wraparound Intensive Services ( WISe ) Program to ensure children and families receive culturally responsive mental health services.

    The

    Care Coordinator for our WISe Program

    coordinates services and planning for children and families referred and screened into the WISe program in

    Tacoma, Tukwila and Pierce County

    areas . As a


    Care Coordinator , you will guide, develop, implement, and monitor individual case plans and Child Family Team (CFT) to ensure client and family voice.

    This role will guide their team through the WISe process and phases and hold the central point of communication between the WISe team and the client.

    Services will be provided almost exclusively in the community:
    in families' homes, foster homes, schools, etc. The primary role is to ensure that all parties work together toward stability for the child and family.


    Responsibilities include:

    direct services to children and families, including care coordinating/linkage functions, maintenance of case records and progress notes, work as a part of a multidisciplinary team, and flexibility to work evenings and weekends.

    program provides comprehensive behavior health supports to youth with complex behavioral and mental health needs.

    The program's goal is to help youth thrive in their homes and communities and assist in helping them meet the goals they have identified.

    Responsibilities

    Build, plan, facilitate and communicate Child and Family Team (CFT) meeting details with WISe team members, clients and families
    Maintain a caseload of clients
    Ensure to incorporate the input and needs of the youth when forming the team
    Establish and sustain an influential team culture by inviting CFT members to propose, discuss, and accept ground rules for working together
    Over time, work to transition facilitation to allow for youth and/or family members to be facilitators of their meetings
    Cultivate and maintain relationships with community members and providers
    Utilize consensus-building techniques to meet the needs of the youth and family
    Provide assistance in case planning and consultation to team members on clinical issues, as well as casework issues, as appropriate
    Maintain team focus on the scope of work for the progress/movement toward transition, acknowledging and celebrate successes and changes
    Stay abreast of all up-to-date Cross System Care Plans for each client and any other related documentation
    Willing to respond to crises and be part of a rotating on-call system
    Qualifications

    Bachelor's degree in Social Work/Psychology/Counseling
    Must be 21 years of age or older
    In person work and required travel throughout Pierce County
    Clearance of TB test, fingerprints, and any other state or federal requirements
    Own vehicle; valid Washington Driver's License and insurability by Seneca's automobileinsurance carrier
    Ability to work as part of and supervise a multidisciplinary team
    Experience in WISe or other WRAP programs
    Strong coordination, organizational and writing skills
    Flexible schedule with ability to work evenings and weekends
    Bilingual skills are preferred
    Proof of COVID vaccination and booster
    Schedule

    Full-time.

    10:00 am- 6:00 pm

    Mon-Fri. Flexibility with daily schedule required.

    Starting salary at $ $26.19 hourly, actual salary is dependent on creditable experience above the minimum qualifications for the role
    $500 sign-on bonus
    Additional compensation provided upon passing bilingual language proficiency exam
