Social Work Firector
 Performs initial psychosocial, emotional, spiritual, and bereavement assessments
and assists in the development and implementation of goal directed IDG care
plan.
 Oversees 5 regions in South Texas to include supervising 4 social workers.
 Provides bereavement and grief counseling to patient and family.
 Utilizes and refers patients to community resources
 Provides short term crisis intervention and individual or family counseling when
indicated.
 Provides ongoing counseling related to issues of death and dying to the patient
and family as needed.
 Patient discharge planning when graduating from Hospice
 Provides education on the death and dying process.
