• Discuss within an interdisciplinary team to establish barriers, strengths, and goals of a patient’s stay within the IRC
• Directed and engaged in meaningful family trainings to support caregivers and patients in achieving their goal of returning home
• Auditing, researching, developing, and implementing strategies to provide effective and efficient culture of hourly rounding to promote patient and caregiver satisfaction, reduce falls and risk for skin breakdown
