    Provides services at the Pediatric and Adolescent Transformative Health (PATH) Clinic.

    PATH is a 12-member collaborative led by Saint Anthony and will provide an innovative structure for a Behavioral Health Home (BHH) serving children, adolescents, and families on the west and southwest sides of Chicago.

    Position Purpose:

    The SDOH Care Coordinator plans, organizes, and implements care coordination services that will address the Social Determinants of Health to prevent re-admission and improve the current health status of patients through linkages to community services and resources.

    Flexible schedule to address program needs including evening and weekend hours required.


    Essential Functions:

    Conducts psychosocial assessments of patients and family members to obtain information about home environment, family relationships, perception of patient#s illness and coping resources to provide emotional support and care coordination assistance.

    Encourages understanding and acceptance of any diagnosis as well as the treatment plan. Gathers pertinent information from medical records, multidisciplinary team, and family to formulate a comprehensive plan of care. Collaborates with patients, families, and staff on care coordination needs and plans for needed services/resources as needed. Documents all interactions and services provided in the client#s case file and manages. Participate in program planning and evaluation activities.

    Is accountable for monitoring, documenting, and tracking program and client data and assuring compliance with grant related documentation requests made by lead care coordinator.

    Participates in performance activities including problem identification, data collection, solution selection, implementation and evaluation. Also assists in the care and maintenance of departmental equipment and supplies as appropriate.

    General Job Requirements:
    Bachelor#s degree in social services or related field Required. Prior experience in care coordination/case management Preferred. Bilingual in English/Spanish Required. Excellent interpersonal skills.

    Requires excellent observation skills, analytical thinking, problem-solving and ability to communicate and interact with all levels of professionals, in addition to good verbal and written communications skills.


    Good critical thinking skills:
    ability to assess, evaluate and teach. Flexible with a working knowledge of all areas of medicine. Ability to adjust priorities quickly, organize multiple tasks simultaneously. Self-motivated and able to work independently and as a team member. Ability to work flexible schedule to address program needs, including some weekend and evening hours.
