    Job Title: Senior Guidance Program Coordinator

    Program: Senior Guidance Outpatient Services

    Location: 700 Airport Road, Lakewood New Jersey

    Position Type: Full Time

    Job Description/Summary

    The Senior Guidance Program Coordinator is a clinician who is familiar with the needs of older adults and how these needs impact mental health. This professional takes a trauma informed approach to care and exhibits an understanding of evidenced based practice. The clinician plays an important role in the expansion and enhancement of services at PBHG by providing direct care to older adults in the community and building relationships with partners in the community who support the same population and their families. This individual participates in the multidisciplinary team and seeks opportunities to learn and develop clinical skills. This person consults regularly with internal departments regarding all areas of senior behavioral health care.

    Responsibilities

    • Submits monthly/quarterly data to county and state offices.
    • Outlines preliminary treatment plans that addresses immediate needs of the client. Recommendations include identification of individual needs and plan outlines how these will be addressed.
    • Facilitates individual, family and group counseling. Begins and ends group on time, prepares any materials as required, initiates new or improved group processes based on needs of the population.
    • Provide program services in locations that meet the needs of the persons served including traditional offices, residences, and community resource sites.
    • Provides case management as needed including timely and effective advocacy, consultation, service coordination, and phone contact to community resources and related professionals. Documentation reflects all contacts made with and/or on behalf of each client.
    • Develops Individualized treatment plans and review of client progress within required timeframes. Problem List specifies problems in current, behavioral/observable terms and reflects integration of needs identified.
    • Collaborates with Treatment Team to identify clients that are at risk for discharge based on lack of contact, creatively engages vulnerable populations, and makes at least 3 attempts to engage client in accordance with policy, and beyond in high-risk cases.
    • Consults with Leadership to determine schedule that meets the needs of the population served.
    • Engages in weekly supervision with OPD Clinical Supervisor, and attends treatment team meetings, including agency-wide clinical case reviews.

    Qualifications

    • Education: Master's degree in social work, mental health counseling, or a related mental health discipline
    • Licensure: NJ licensure as LSW, LCSW, LAC, LPC, LAMFT, or LMFT
    • Experience: Working with older adults with a wide range of emotional and behavioral need
    • License: Valid Driver's License in good standing

    Competencies:

    • Possesses strong organization and time management skills.
    • Communicates and works effectively within a team
    • Ability to communicate in English, bilingual in Spanish is a plus.
