    Description

    Salud Para La Gente (SALUD) provides high quality, comprehensive and cost-effective full scope primary care services to underserved, low-income communities in the Monterey Bay area, including Santa Cruz and Monterey Counties. SALUD began in Watsonville in 1978 as a storefront "free clinic," and with a growing need for health services became a Federally Qualified Health Center (FQHC). Today SALUD is a primary health care network with four primary clinic sites, school based sites, and over 400 employees. We provide high quality services to patients of all ages.     We are seeking a dynamic Care Team Nurse to work within the clinical care team providing support in the delivery of patient care as well as direct patient care. The Care Team Nurse utilizes nursing skills and judgement to triage patients and provide patient education, along with providing direct patient care such as medical procedures, immunizations and medication delivery. Additionally in collaboration with the Associate Director of Nursing, the Care Team Nurse provides supervision and guidance to certified medical assistants within the care team. The Care Team Nurse demonstrates critical thinking skills and leadership, and models team based care.

    Responsibilities include but are not limited to the following:

    1. Utilizes the nursing process (assessment, planning, intervention and evaluation) in patient care and daily activities. Provides comprehensive assessment and utilizes evidence based guidelines, along with insight of the nursing process, and a proven ability to critically analyze information from the patient and the patient chart in the decision making process.
    2. Functions under standardized procedures or patient care protocols to provide independent patient care.
    3. Administers medications and immunizations within scope of practice and with clinician's order.
    4. Provides direct patient education, explains medical procedures, treatments, and medications to patients and families, and supervises the provision of patient education by certified medical assistants. Communicates based on patient's level of understanding and comprehension. Educates patients utilizing principles of active listening.
    5. In collaboration with the Associate Director of Nursing, supervises clinical care provided by certified medical assistants as assigned and trains certified medical assistants for clinical processes as directed.
    6. Performs independent telephone or face to face triage using written triage protocols.
    7. Under written orders, provides test results to patients and advice on treatment or follow-up.
    8. Assures efficient patient flow by ongoing assessment of clinician schedule, and working with other staff to minimize patient waiting time.
    9. Functions as Clinic Supervisor when assigned.
    10. Participates in staff meetings.
    11. Maintains confidentiality and monitors confidentiality policy adherence for certified medical assistants and other support staff.
    12. Consistently adheres to and/or exceed Salud's communication guidelines and expectations with internal and external customers including co-workers, patients, and external partners.
    13. Communicates effectively by using welcoming words, proper tone of voice, appropriate body language, eye contact and smiling in all interactions. Listens skillfully and displays a willingness and ability to acknowledge patient and co-workers' needs, expectations and values through the use of reflective listening and empathy conveyance. Responds to co-worker and patient needs in ways that are helpful and beyond expectation.
    14. Performs other duties as assigned.
    Requirements

    MINIMUM QUALIFICATIONS:
    • Current California RN license and good standing with the Board of Registered Nurses.
    • Completion of an accredited RN program.
    • At least one year experience in acute care nursing
    • Experience in ambulatory clinical setting and/or Federally Qualified Health Center strongly preferred.
    • Minimum of one year's experience in acute care nursing strongly preferred.
    • New nursing graduates considered with at least one year prior supervisory experience.
    • Valid CA Driver's License and current vehicle insurance; must be able to travel between sites.
    • Bilingual English/Spanish strongly preferred.
    • Current CPR
    PHYSICAL DEMANDS:
    • Standing, walking, sitting, typing, reaching, moving and/or lifting up to 50 pounds.
    SALARY & BENEFITS

    Salary: $97,741.85-$118,805.80 per year
