    Job Summary :

    The Research Care Coordinator works with a team of community health workers to provide care navigation, care linkage and connection with community based resources to pregnant and postpartum patients (and potentially their families) to address the social drivers of perinatal.

    S/he will assist in

    development/implementation

    of policies, programs and procedures, and participate in staff development.

    Specifications :
    Master's Degree Required, LICSW Preferred. Knowledge of current perinatal health systems, care navigation and community-based
    Care New England Health System (CNE)
