    Wellness Nurse

    Department: Nursing Services/Resident Services FLSA/Classification: Exempt/Salary Reports to: Director of Resident ServicesDirect Reports: NA

    Summary:

    Responsible for the clinical management and direction of the Independent Living areas under state and federal regulations. The Wellness Nurse also serves as a resource for the entire community regarding residents' cognitive and functional ability as this relates to living in the community. Performs other duties as assigned.

    Essential Duties and Responsibilities include, but are not limited to:

    Leadership

    • Works collaboratively with the community "Leadership" team to ensure success in meeting and exceeding community goals in operations, human resources, sales, and resident care.
    • Responsible for protecting and advocating for resident rights.
    • Implements community initiatives within the care venues as directed.
    • Maintains gerontologic nursing competencies and increases personal growth and development through formal and informal education programs.

    Clinical, Operational, and Quality Improvement Responsibilities

    • Directs resident care and coordinates services given by other care providers.
    • Manages practice so it is consistent with current standards promulgated by the company and appropriate local, state, and federal regulatory bodies.
    • Establishes and sustains quality assessment/performance improvement activities.
    • Identifies and reports deviations from safe practice. Adheres to policies and guidelines of regulatory agencies (i.e., OSHA).
    • Manages emergencies based on the community's safety and disaster policies.
    • Assists the DRS with completing and/or receiving initial and ongoing assessments of residents for appropriate placement within the community.
    • Communicate with company-level resources regarding pertinent resident care issues as needed.
    • Assists the DRS with working with residents, family members, and staff to ensure needs are met in a timely and efficient manner.
    • Responsible for the coordination of all clinical services between the resident, personnel, associates, family members, and contracted services.
    • Oversees the quality of care provided, including monitoring both individual cases and care trends. Identifies and reports any care issues or trends to the Director
    • Acts as a clinical resource to all associates.
    • Participates in Resident Review Meetings and Discharge Planning meetings. (PHI)
    • Assists with clinical lab requests for IL residents.
    • Manages the "Here to Help Program".
    • Conducts the "Blood Pressure Checks" program within the community.
    • Assists and responds to pendant activations for IL residents.
    • Provide clinical guidance on the medical evaluations of prospective clients working with the Sales & Marketing team.
    • Conduct vaccine clinics for the community.
    • Other duties as assigned.

    Regulatory Compliance

    • Ensures that all associates providing care maintain all professional standards of nursing practice in accordance with local, state, and federal regulations and accrediting bodies.
    • Participates in the development, revision, and implementation of policy and procedures for the delivery of services.

    Key Competencies:

    • Must possess knowledge of regulatory compliance and continuing Care Retirement Community operations.
    • Maintains a courteous and professional manner through interactions with others.

    • Uses discretion in handling confidential information, incorporating all federal, state, and local privacy and confidentiality requirements.
    • Must be sensitive to the needs of older adults and enjoy working with a senior population.
    • Excellent oral and written communication skills and ability to interface and maintain effective relationships with all departments and employees in a team-oriented environment.
    • Computer proficiency using Microsoft Office.

    Minimum Education/Skills/Experience/Credentials:

    • Registered Nurse or LPN in good standing
    • Must possess and maintain an active and current license to practice in the State of Illinois.
    • One to three years of experience in a hospital, long-term care, or other healthcare-related facility.
    • Must have good interpersonal relations and proven verbal and written communication skills.

    Physical Requirements:

    • Must have the ability to stand and walk for 4-5 hours of an 8-hour shift.
    • Must have the ability to sit for 3-4 hours of an 8-hour shift.
    • Must have the ability to occasionally stoop, bend, and kneel.
    • Must have the ability to occasionally lift 75 to 100 pounds while bending at the waist and knees while assisting in resident transfers.
    • Must have the ability to occasionally push, pull, or move medical carts, residents using lift systems, and residents in wheelchairs exerting a force of 40 to 100 pounds.
    • Must have the ability to use wrists, fingers, and hands for repetitive firm grasping.

    Working Conditions:

    • Available for Emergency calls
    • Regular on-site physical presence is required
    • Reliable, predictable, and punctual attendance to provide effective management of staff, provide timely service to internal and external customers, and support uninterrupted business operations and continuous medical care to residents is required.

    Other federally regulated requirements:

    • Must be willing to undergo pre-employment background checks, including (MVR) moving vehicle records.
    • Must have or be willing to receive the Flu-Shot Vaccine and TB testing.
    • Must be willing to undergo pre-employment drug screening, including cannabis.
    • Must be willing to undergo biometric screening.
