    Summary:


    The Service Coordinator II with Child and Family Connections assists in determining eligibility for designated services and enables families to receive their rights, procedural safeguards, and services that are authorized under the state's early intervention program.

    The service coordinator builds relationships that include providing information to families and coordinating information among multiple service providers.

    The service coordinator maximizes the use of existing resources and refers families to any other needed services, tailoring the unique circumstances of each individual child and family as well as coordinating complex referrals independently.

    The service coordinator exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

    Required Job Qualifications:

    • Bachelor's degree in a related human service field such as early childhood special education, social work or psychology
    • 2 year of experience in the Service Coordinator I role
    • Demonstrates knowledge of the developmental, physical, psychosocial, medical and cultural needs of children and families.
    • Demonstrates excellent organizational and case management abilities.

    Responsibilities:

    • Coordinates with specialized team/s on referrals for specific vulnerable populations including but not limited to: deaf/hard of hearing and visually impaired children, refugees, youth with DCFS involvement, medically complex referrals, children residing in group and medical homes, families experiencing homelessness, and less frequently spoken languages.
    • Provides information to families on the early intervention system in the state and local area, including their rights and availability of services.
    • Assists families in identifying available service options, including the availability of advocacy services.
    • Gathers all pertinent medical, developmental and social information for the child and family and coordinates the performance of evaluations and assessments in a timely manner.
    • Facilitates and participates in the development, review, evaluation and maintenance of Individualized Family Service Plans (IFSP).
    • Coordinates and monitors the ongoing delivery of services to ensure that all resources are being utilized and the IFSP goals are being addressed.
    • Gathers all pertinent family insurance and income documentation and assists families in understanding fee and insurance usage in the early intervention system.
    • Maintains the child's CFC record in a clear, complete format following DHS and Rush policy, including correspondence and case notes.
    • Facilitates the development and implementation of transition plans, including school or other services as appropriate.
    • Serves as mentor to Service Coordinator I through "shadowing" IFSP intakes and IFSP meetings, debriefing of those appointments, explaining post IFSP steps and serve as a resource for less experienced Service Coordinators.
    • Communicates effectively within the hospital and department, as well as with families, community providers and the Department of Human Services.
    • Responsible for personal and professional growth and development.
