    Job Description: Medication Coordinator

    As a Medication Coordinator, you will work under the supervision of the Director and Clinical Supervisors at Tres Puentes. Your main goal will be to supervise residents in self-administering medications to ensure their safety and independence.

    • Resident Medications/Supplies:
      • Medication Administration: Administer medications according to orders and regulations.
      • Medication Storage: Ensure proper storage of medications.
      • Medication Orders: Dispense medications only with written orders.
      • Documentation of Medication Orders: Keep accurate records of medication orders and administration.
      • Documentation of Medication Administration: Document all medication administrations in real time.
      • Medication Supply: Maintain an adequate medication supply.
      • Medication Changes: Ensure proper procedures for medication changes.
      • Incontinence Supplies: Assist residents with incontinence supplies.
      • Medication Updates: Manage medication updates and appointments with providers.

    Additional Responsibilities:

    • Assist residents in activities of daily living.
    • Other:
      • Collaborate with the team to provide high-quality care.
      • Communicate effectively with staff and attend training sessions.
      • Submit required reports promptly and assist in other duties as needed.
      • Provide personal care support to residents.

    Qualifications Needed:

    • Minimum of 2 years' experience working with dependent persons.
    • Must be at least 21 years old.
    • Ability to work in a multi-cultural environment.
    • Physically and mentally capable of performing job duties.
    • Proficient in English.
