Assess medical needs of residents and assure that necessary care and services are being provided.

Maintain accurate records of care and assure all physicians, families or surrogates are notified of any resident condition changes.

Administer medications, performed prescribed treatments and document all necessary information as needed and assure that all physicians' orders are followed as written.

Respond promptly to all emergency calls and other requests in the Community for medical services and document all changes and pertinent resident information.

Arrange for home health care and services, if needed, upon resident's discharge from HCC.

For purposes of effective communication with residents, families and staff must be able to see, hear, speak, read and write English.
