- Facilitate access to primary care and behavioral health providers
- Collaborate with interdisciplinary teams to ensure effective care management
- Engage members in therapeutic interactions to promote health behaviors
- Coordinate care, including resource linkages and seamless transitions
- Provide in-person care management and care coordination services
- Deliver health education and support services to members
- Participate in systematic case reviews and ad hoc case reviews
- Follow company policies and procedures for quality care
- Education: High School Diploma/GED, Associates Degree (Preferred)
- Experience: 2 years' experience as an LVN in a healthcare setting
- Language: Fluent in English (Bilingual preferred)
- License/Certification: Valid state LVN license, BLS, and CPR certifications
- Skills: Computer proficiency, communication skills, problem-solving abilities
- Operate office equipment, travel to provide services in the field
- Ability to lift up to 15 pounds and travel to different locations

About the Role
The Nurse Care Coordinator role is a key position within our ECM team. Enhanced Care Management (ECM) is a program funded through California Advancing and Innovating Medi-Cal (CalAIM) that focuses on providing personalized care to individuals to improve their overall health and well-being. As a Nurse Care Coordinator, you will work closely with healthcare providers and community organizations to coordinate care and support services for members enrolled in the ECM program.
