    Acute Rehab/Med Surg

    Schedule:
    Day, 36hrs/wk, 7am-7:30pm with every other weekend commitment and holiday requirements as well

    *Travel, PRN, Short-Term Option, etc.). Provide basic bedside care to long term care patients under the direction of a Registered Nurse
    (Contribute to the assessment of patients by collecting data for analysis by a Registered Nurse (RN).

    Educate patients/families about disease treatment plan including self-care post discharge, holistic health needs, available resources and follow up care.

    Assure resident safety by identifying safety hazard and emergency situations and initiate corrective action as appropriate.
    Licensure / Certification / Registration:

    Licensed Practical/Vocational Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.

    Previous experience in long term care setting preferred. Ability to prioritize and flex throughout the fast paced shift. Able to take an assignment, pass medications, document data collection in an accurate and timely manner.
