    We are seeking a highly motivated, experienced Board-Certified Family Nurse Practitioner or Physician Assistant for immediate, full-time placement at its sites in Brooklyn.

    Auburn Assessment Center 39 Auburn Place Brooklyn, NY 11205 Mondays, Wednesdays and Fridays.


    Legacy:
    Avenue D Brooklyn, NY 11236 Tuesdays and Thursdays.

    Under the supervision of the Chief Medical Officer, primary responsibilities include:


    • Provide primary care.
    • Perform well-care examinations.
    • Distribute medications to patients.
    • Communicate with case workers.
    • Encourage health promotion, disease prevention, and health maintenance.
    Requirements

    Education:
    Candidate must possess a valid NYS Family NP or PA license and must be Board Certified.

    Experience:
    Must have experience treating patients of all ages.


    • Prior experience with electronic medical record (AthenaHealth preferred) a must.

    Language:
    Bi-lingual (Spanish) desirable.

    Salary Description

    $125,000 Annually pl as applicable.
