    Adult Support Coordinator

    Schedule: Monday - Friday 8:30a to 5p

    Support Coordination assists individuals with developmental disabilities and their families in accessing services, supports, and resources within their community, utilizing a person-centered approach. Support coordination functions as the entry to access needed community resources and services.

    The Adult Support Coordinator ensures continuity of directly provided and contractual services to the agency's individuals with intellectual and developmental disabilities. Support Coordinators assist individuals with their basic physical, financial, social, and personal needs. Support Coordination services for individuals include the development of person-centered plans, service provider linkage, monitoring, advocacy, counseling crisis intervention, daily living support, and enhancing community integration.

    Requirements: Bachelor's degree in Human Services (Social Work, Psychology, or Counseling) with one (1) year of experience in support coordination services with the assigned population. Valid Driver's License.


    The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
