At WelbeHealth, each participant in our program is guided by our interdisciplinary team (IDT) - a blend of clinical and non-clinical members collaborating to deliver top-notch patient-centered care. As a crucial member of the IDT, the Physical Therapist plays a pivotal role in care planning and teamwork with the clinical squad.

The focus of the Physical Therapist is to coordinate and offer restorative and rehabilitative Physical Therapy services to participants both at the Center and their homes. Together with the Primary Care Physician, rehabilitation team, and other IDT members, the Physical Therapist strives to enhance participant independence, safety, and functional abilities.

Key Responsibilities:

Conduct assessments in our facility and participants' homes to identify Physical Therapy care needs, preferences, and goals
Actively engage in IDT meetings to devise participant care plans
Implement and document Physical Therapy interventions as per physician orders and participants' care plans
Provide various treatments such as exercise, muscle re-education, gait training, and ADL functional training
Supervise Therapy Assistants, offering necessary training and guidance for Functional Maintenance Programs (FMP)
Deliver training to staff, participants, and caregivers on therapeutic equipment use and maintenance

Minimum Requirements:

Bachelor's Degree in Physical Therapy with at least five (5) years of experience
Master's Degree in Physical Therapy or Doctorate in Physical Therapy highly preferred
Two (2) years of experience in a clinical setting with frail or elderly populations is a plus
Evaluation of any physical therapy degree earned abroad to its US equivalent
Current CPR certification is preferred

Salary/Wage range for this role falls between $99,309.58 to $119,171.49 USD annually, plus Bonus and Equity. WelbeHealth guarantees an attractive total rewards package inclusive of 401k match, healthcare benefits, and various other perks. Actual salary will be adjusted based on candidate experience and qualifications.
