The Home Health Aide is a member of the home care team who works under the supervision of a registered nurse and performs various personal care services as necessary to meet the patient's needs.
The home health aide is responsible for observations and documenting observations and care performed.The home health aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care.
Responsibilities of the home health aide are outlined in the plan of care, including but not limited to:Skin care to prevent breakdown
Assisting in ambulation and exercise according to the plan of care
Performing range of motion and other simple procedures as an extensional therapy service as ordered
Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)
Adhering to the Agency's documentation and care procedures and standards of personal and professional conduct
Maintains a patient case load, daily visits and point of care documentation levels as per Agency standards
Maintains privacy and confidentiality with regard to all patient, staff and agency information
You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.
Current Certified Nursing Assistant certification issued by the Texas Health and Human Services and a person who has successfully completed a State-established or other training program that meets the Texas requirements~ 1+ years of experience in health care
~ Experience working with electronic medical record application
~ Proficient in English with written and oral communication
~ Thorough knowledge of current Nursing Aide practice, professional standards of care, and state and federal regulations regarding home care
~ Demonstrates excellent observation, verbal and written communication skills, problem solving, basic math skills, and nursing skills per competency checklist
~ Must not register as unemployable on the Texas Nurse Aide Registry nor the Texas Employee Misconduct Registry
Experience in home health care
