    Social Worker (LMSW) Opportunity

    Are you a Licensed Master Social Worker (LMSW) looking for a fulfilling role in the Cincinnati area? Constellation Health Services is seeking a dedicated and compassionate LMSW to join our team and provide care to hospice patients. In this position, you will have the opportunity to work directly with patients in their homes and at Skilled Nursing Facilities.

    Who We Are Looking For:

    • Must hold an LMSW license in Ohio with a minimum of 1 year of clinical experience. Experience in areas such as Hospice, Acute Care, and Bereavement Counseling is highly valued.
    • Having reliable transportation and being open to commuting within a specific territory to deliver top-notch in-home patient care.

    What You Will Do:

    • Conduct clinical social work for our hospice community, performing social assessments and developing intervention plans based on findings.
    • Provide education to patients and their families on medical conditions, treatment plans, and medication management.
    • Ensure accurate documentation and timely submission of all records using our electronic medical record system.
