- Provide skill-building services using a community-based approach, both in-person and via telehealth.
- Conduct thorough assessments and develop individual service plans with clients and families.
- Monitor goals and client progress, making adjustments as needed.
- Establish effective communication with internal and external teams.
- Assist clients in applying for community benefits and resources.
- Submit accurate and timely documentation.
- Maintain productivity standards and confidentiality.
- Participate in supervision and accept feedback constructively.
- Exercise good judgment in all duties and responsibilities.
- High School Diploma and 2 years of mental health experience required; Bachelor's degree preferred.
- Proficiency in American Sign Language (ASL) is a must.
- Experience in providing services to improve social, vocational, and mental functioning.
- Availability of a personal car and valid driver's license.
- Effective oral/written communication skills.
- Computer proficiency and record-keeping skills.
- Ability to work varied hours, including evenings and weekends.
