    Job Description

    Bachelors Degree required, this position requires a minimum of two years of professional experience in juvenile justice, child welfare, behavioral health or related field. The applicant will also need to have the necessary skills and capabilities to use a computer, facilitate groups, work a flexible schedule, valid drivers license, approved driving record, and their own vehicle to carry out the demands of the position. There is significant travel in this position.

    The Community Services Program is a continuing partnership with the Department of Youth Services and the Old Colony Y. The program works with DYS committed youth returning to their home community from a residential or treatment program. The DYS youth are supervised within this program to prevent continued delinquent behavior by building skills, providing opportunities, and enhancing individual and family strengths.

    The primary role of the Youth Service Coordinator (YSC) is to work closely with the case management team to ensure that youth are connected to services which align with their identified needs, strengths and assessed level of risk. The Youth Service Coordinator will support the casework team in ensuring that the most intensive monitoring and interventions are applied to those at highest risk for reoffending. The Youth Service Coordinator will balance the intensity of his/her involvement in cases in coordination with the case management team concentrating on matching needed services of all youth to their specific dynamic risk factors.

    Duties include:
    • Identifies, coordinates and sustains relationships with community-based organizations and providers to ensure a menu of services and resources is established to meet the individual needs of each youth and family.
    • Conducts home visits, monitors youth curfew, discharge planning, school visits, employment checks, and attendance checks
    • Identifies pathways for youth and families to access benefits and services available to them through state agencies/benefits providers
    • Plans and executes pro-social services activities and events such as recreational outings, community service projects and family engagement activities
