    CenterWell Senior Primary Care* is seeking a Cardiology Nurse Practitioner in Houston, TX Centers

    As a Cardiology Nurse Practitioner with CenterWell Senior Primary Care, you have the opportunity take a hands-on approach to population addressing patient needs inside the exam room and look beyond to understand how social determinates of health are impacting health outcomes.

    *A day in the life of a CenterWell Cardiology Nurse Practitioner
    • Thrive in a value-base care model and improve patient's quality of life
    • See a variety in your caseload, from preventative care to chronic conditions
    • Assists in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment and home health care
    • Acts as a lead and active participant and key source of medical expertise with the care team through daily huddles
    • Follows policy and protocol defined by Clinical Leadership
    • Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care
    *Basic computer skills, familiar with EMR and email

    • Current, unrestricted Drug Enforcement Administration (DEA) registration
    • Bi-lingual a plus
