Job Description

Salary: $15-$22 per hour

Job Purpose:

Blair Family Solutions, LLC is committed to offering coordinated treatment through the Blended Case Management Program. This program aims to empower individuals with Mental Illness or emotional disturbances and their families by helping them access necessary services and expand their worldview. As a Case Manager, your role involves assessing needs, providing support, and advocating for services to help clients achieve their goals.

Duties and Responsibilities:

Complete client assessments to identify service needs
Develop individualized Service Plans
Assist clients in obtaining necessary services
Monitor and follow up on service implementation
Coordinate safe and effective care
Advocate for clients' needs and intervene in crises
Support clients in decision-making and transition planning
Record case information accurately and adhere to professional standards
Enhance organization reputation by adding value to job accomplishments

Case Manager Skills:

Empathetic and a team player
Effective communication and organizational skills
Problem-solving abilities and compassion
Must possess a valid license

Qualifications:

Bachelor's Degree in related fields or equivalent experience
High school diploma with relevant coursework and experience

Working Conditions:

Work may vary in settings like clients' homes, schools, or offices

Physical Requirements:

Includes oral and written communication, observation, sitting, standing, walking, driving, and occasional lifting
