    Company Summary: Crossroads is an Arizona Department of Health Services licensed substance abuse treatment provider with proven expertise in serving men, women and Veterans with Substance Use Disorder through the provision of high quality, successful, and affordable residential and outpatient recovery programs.

    Due to federal contracts the following items are prohibited: Medical/Recreational marijuana, current parole or probation.

    Job Summary: The Case Manager supports and assists the Clinical Coordinator in all aspects of client services, including treatment planning, case load management, group/individual interventions, crisis management, coordination of care, and discharge planning.

    Supervisory Responsibilities:

    • None

    Essential Functions:

    • Develops and provides individual treatment plans; monitors, evaluates and documents client's progress.
    • Maintains a client caseload, set by manager, completes all required documentation in a timely manner, including progress notes, evaluations, treatment plans, coordination of care, and discharge planning, all in accordance with regulatory requirements and Crossroads policies and procedures.
    • Provides individual and group intervention to include crisis management as required.
    • Facilitates coordination of care to include case review, weekly progress updates in treatment and coordination of aftercare services.
    • Participates in clinical staff meetings, trainings and other activities as required.
    • Discusses issues regarding program requirements and/or conflicts with clients.
    • Assist clients in completing applications for government assistance, probation/court obligations, and referrals to independent agencies.
    • Adhere to internal policies and procedures and behavioral health licensure, local, state, and federal rules and regulations.
    • Performs other duties, as required.

    Qualifications:

    • Must meet Arizona Department of Health Services personnel member requirements of: at least 21 years old; or licensed or certified under A.R.S. Title 32 and providing services within the personnel member's scope of practice.
    • Must maintain high ethical standards and demonstrate sound judgment, problem solving skills, and strong organizational skills.
    • Valid Fingerprint Clearance Card.
    • Must be proficient in Computer/Microsoft Office/EMR and typing.
    • Current CPR/First Aid/Narcan Certification.
    • Current and annual negative TB test.

    Knowledge, Skills and Abilities:

    • A working knowledge of addiction, recovery, and twelve step programs.
    • Well-developed written and verbal communication skills and ability to interact effectively with internal and external audiences.

    Education & Experience:

    • Minimum of High School diploma or GED required.
    • 1-2 years related experience and/or training or equivalent combination of education and experience required.

    Work Environment:

    • Fast paced behavioral health setting.
    • Primarily indoors, however, some outdoor work may be requested.

    Physical Demands:

    • While performing job duties, the employee is regularly required to sit and speak or hear. The employee is occasionally required to stand, walk, use hands to handle or feel, reach hands and arms, stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move up to 50 pounds and seldom lift and/or move >100 pounds. Specific vision abilities required by this job include color vision, depth perception, and ability to adjust focus.

    Shift Requirements:

    • Days, nights, weekends and holidays.

    Travel:

    • Valid AZ Driver's License.
    • May include travel between sites and other locations.
    • Local and state wide travel when requested or required.
