Overview
The Patient Care Coordinator (PCC) LVN is primarily responsible for patients' clerical and communication care coordination from admission to discharge.
Performs a wide variety of clerical and if applicable clinical duties for the Pediatric and Palliative Care programs assigned to including, but not limited to, processing and maintaining patients' clinical records, Interdisciplinary Group (IDG) prep, processing and tracking of physicians' orders, processing patient authorization requests, certifications, and ensuring seamless communication between patients, staff, and team manager.
Serves as key communication conduit for Pediatric and Palliative Care programs, the Medical Group within the agency and for outside communication.

To enhance the quality of life for those nearing the end of life's journey and for those who grieve.
Work Schedule:
Monday through Friday, 8:00 a.m.-5:00 p.m., on-call availability preferred. (On-call schedule: 1 week day NOC shift from 5pm - 8am per week and 1 weekend day DAY or EVENING shift per month.)
Salary:
$32 - $36 hourly. Subject to experience and qualifications.

Travel throughout San Diego County may required for patient visits, training, or office visits.
Responsibilities
Generates and monitors staff data integrity reports for staff compliance and evaluation of performance and quality measures including Patient Task Report, Unverified Services Reports and other reports as requested.
Generates, processes and tracks all physician recertification's and Do Not Resuscitate (DNR) / Physician Orders for Life-Sustaining Treatment (POLST) to ensure timeliness and accuracy.
Prepares correspondence, reports, and materials as requested.Responsible for assisting with various clerical duties such as scanning, faxing, copying, entering notes, demographics, and insurance information into electronic medical records.
Assist with Palliative Care and Pediatric Program referral process, including opening charts and obtaining insurance information for both populations.Creates, transcribes, and distributes meeting agendas and minutes.
Maintains a calendar of activities, meetings and various events for the department, coordinates activities with other departments.
Maintains the department's general files in hard-copy and electronic filing systems.
Acts as back up support for other departmental roles, including but not limited to, Palliative Care Coordinators and Physician / Nurse Practitioner Scheduler.
Provides back-up support to other PCC staff as needed.Works in collaboration with the medical records staff to ensure all orders printed by Medical Records are presented to the Provider at IDG meeting for completion as needed.
Compiles, updates, and maintains components of the clinical record from admission to discharge including changes in patient status, level of care, location, mortuary, CDAP and family and friends' resource if requested by qualified field staff or clinical leader.
Prepares correspondence and reports for team and other requests such as therapy referrals and packets for NP/MD visits.Takes incoming calls from patients, caregivers, and facilitates professional, compassionate communication with staff, patient, and family. Responds to questions in a timely manner, including use of tiger text when appropriate.
Acts as a liaison between mortuaries and hospice physicians to ensure death certificates are completed in an accurate and timely manner.
Updates mortuary, friends, and family resources in EHR as requested by IDG.Requests Health & Physical (H&P) and inpatient records from hospitals and Skilled Nursing Facilities (SNFs).
Add and Work with hospice physicians to resolve and clarify codes and diagnoses with conflicting, missing, or unclear information, runs note audit reports.
Prepares materials for and coordinates set up for IDG meetings including the Part D Covered/uncovered Query for MD review during meeting.
Generates, scans, and faxes Patient Status Reports to community physicians.Generates staff bereaved lists and processes sympathy cards.
Monitors and tracks clinical staff Unscheduled Paid Time Off (USPTO) in Excel Staff Schedule.
Performs special projects or other assignments as requested by Clinical Manager.
Reviews monthly reports on medical supply use and ensures the medical supply closet is stocked, organized and does not have expired items available for use by staff.
Performs LVN visits PRN as designated by the manager.Takes orders from physicians if requested to help facilitate patient care.
Maintains Biohazard waste cabinet and tracking forms
Qualifications
High School diploma or equivalent required and a minimum experience of 3-4 years in healthcare field.
Current LVN license required.
Working knowledge of medical terminology required.
Working knowledge of HIPAA regulations.
Demonstrates proficiency in typing skills and use of Microsoft Office programs; ability to learn new software applications.
Possess excellent customer service skills.
Demonstrates ability to communicate effectively with staff, patients, physicians and community agencies, both verbally and written.
Ability to manage time efficiently.
Work requires attention to details, excellent organizational skills and discretion with confidential patient information.
Requires professionalism and experience to interface with all levels of management and personnel.
Bilingual preferred.
Current CPR certification and negative TB skin test.
Pediatric and Palliative Care experience preferred.
Current California driver's license and proof of auto insurance.
