    PACE (Program of All-Inclusive Care for the Elderly) was developed to answer the many problems around caring for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.

    Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care, social and recreational activities, and other senior care services.

    POSITION SUMMARY: Under the clinical direction and supervision of an On Lok Physician and the administrative direction of the Associate Medical Director, the Nurse Practitioner collaborates in the care of all On Lok participants.

    DUTIES / RESPONSIBILITIES:

    1. Participates in the health assessment and examination of On Lok applicants and participants.
    2. Evaluates the needs and participates in the development of a comprehensive program and nursing treatment for each participant in the home and at the On Lok Day Health Centers.
    3. Manages minor illnesses, injuries, and the stable, chronic illnesses of participants, according to standard procedures.
    4. Evaluates the acute illnesses of the participants.
    5. Coordinates appropriate referrals to health care providers in consultation with On Lok physician.
    6. Makes nursing home/or home visits when appropriate and as directed by the On Lok physicians.
    7. Provides clinical assistance to On Lok's staff registered nurses.
    8. Participates in all programs of health restoration, prevention, promotion and maintenance of health.
    9. Attends and participates in staff meetings.
    10. Fulfills all administrative requirements, including statistical records, chart entries, etc.
    11. Under general supervision, will perform operational level duties.
    12. The major emphasis of this role is ongoing direct patient care delivery.
    13. Assignments primarily encompass management of patients who present chronic and/or episodic health problems, and may include participation in program planning and development of patient education information.
    14. Panel size is a minimum of 60 participants or 70% of MD panel.
    15. May precept NP students.
    16. "On Call" responsibilities.
    17. Other related duties as assigned

    QUALIFICATIONS (knowledge, skills, abilities):

    1. Licensed RN and NP, State of California
    2. Meet standards of Board of Registered Nursing and the appropriate Business and Professional Code, State of California
    3. Successful completion of a program of study, which conforms to Board of Registered Nursing standards
    4. Willingness to work within the framework of a multidisciplinary team in the On Lok health delivery system
    5. One year of nursing experience at the RN level and some supervisory experience.
    6. Two years of experience as a primary care Nurse Practitioner required.
    7. One year experience working with the elderly or frail population required.
    8. Bilingual ability may be required for specific assignments

    Your final compensation offer will be determined based on factors such as skills, experience, education, and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.
