    Description
    General Responsibilities


    This position assumes the primary role in providing and monitoring the healthcare and health maintenance of clients assigned by Melmark for a designated nursing line.

    The Primary Nurse works collaboratively with other members of the multidisciplinary treatment team for case management including the School Nurse, teacher's and teacher's aides, residential staff, health services manager, behavioral analysts and rehabilitative services including PT, OT and Speech therapists.

    The Primary nurse is the key interface with Primary Care Providers, pharmacy, medical insurance companies and medical equipment providers. Must possess an ability to maintain effective communication with families, other nursing staff and the interdisciplinary team.

    The Primary Nurse is responsible for participation in client meeting including client reviews, IEP/IPP's, inter-agency meetings for the coordination of comprehensive medical care and services.

    Starting Rate: LPN $30.00/hour (+) based on years of experience

    Schedule: 7am-7:30p


    Week 1:
    Friday, Monday Tuesday

    Week 2:
    Thursday, Friday, Monday, Tuesday (pick 3)

    Week 3:
    Saturday, Monday, Tuesday

    Week 4:
    Friday, Saturday, Sunday, Monday (pick 3)



    Basic Qualifications:



    Education/Training:
    Licensed LPN from an accredited school


    Certifications/Licensure:
    Current licensure as a Licensed Practical Nurse through the Pennsylvania Board of Nurse Examiners. Current CPR and First Aid certification and 24 hours of in-service training yearly. Successful completion of employer provided certification in CPR, First Aid, Medication Administration and Crisis Intervention.

    Must have held a US Driver's License for a minimum of two years and must be at least twenty-one (21) years old.

    Ability to meet approved Melmark driver status preferred.

    Skill(s): Nursing skills gained through education, experience and training. Good computer skills essential including use of Word processing and data management tools. Past experience with electronic medical records a plus. Must be comfortable with g-tube and j-tube dependent clients.

    Experience:

    A minimum of one (1) year experience in related field preferred; however, over two (2) years' experience is preferred.
