Our client is seeking a Home Health Registered Nurse Case Manager to join their extraordinary care team. This position supports patients in Shippensburg, PA and surrounding areas.

Key Responsibilities
Lead and collaborate to provide management and delivery of patient care plans
Support patients in Shippensburg, PA and surrounding areas
Eligible for a $15K first year success bonus to reward the accomplishment of key achievements during the first year
Schedule and Shift Details
Mon-Fri 8am-5pm; on-call rotation is only 1 weekend every 4-8 weeks depending on staffing. When it is their scheduled weekend, they are on-call Saturday & Sunday, 8am-5pm only for SOC visits. They do not do on-call during the week or after hours.
