    General Summary of Position

    MedStar Health is looking for a Community Health Program Coordinator to join our team at MedStar St Mary's Hospital

    As a Community Health Program Coordinator, you will support the day-to-day coordination, administration, community engagement, and data functions for the Population Health initiatives, under the direction of the Director of Population and Community Health.

    Primary Duties:
    • Acts as primary agent for the coordination and implantation of the Community Health & Wellness program. Assists Case Management and other hospital departments to implement outreach efforts that engage patients to adhere to care plans and/or remove barriers to care.
    • Oversees workforce staff including Community Health Advocates and Drivers. Assists in the synthesis of data analysis into clear, relevant, and visually appealing materials that can be used to make strategic decisions.
    • Builds and maintains relationships with community partners. Coordinates grant applications, implantation, and reporting for CDC and HSCRC (Health Services Cost Review Commission).
    • Coordinates and schedules meetings, conferences, classes, seminars, community activities and other related activities for the department. Ensure meeting readiness to include but limited to room availability, invitations, meeting requests and minutes, and communications. Assists in preparation to include agendas and minutes.
    • Collaborates with various hospital departments to capture and validate community benefit qualifying activities; establishes completion deadlines and documentation standards. Serves as a liaison between communities, individuals and coordinated healthcare organizations. Builds and maintains relationships with community partners.
    Qualifications:
    • Bachelor's degree in public health, Nursing, Social Work, or related field.
    • 1-2 years' work experience including 1 or more years of proven case management, project management and population health familiarity with the local area and/or population health workforce integration.
