    JOB TITLE: Medical Social Worker (M-F - Albany, NY)

    Reports to: Director of Patient Services

    Salary Range: $65,000-70,000

    Job Summary:

    The Medical Social Worker is a qualified professional person who provides medical and social services in accordance with applicable state law and regulation, to patients in the home with physician's or authorized practitioner's orders and under the supervision of the Director of Nursing.

    Ideal Candidate:

    • Graduate of a school of social work, approved by the Council of Social Work Education, with a master's degree.

    • One (1) year experience in a medical facility (clinical, hospital, rehabilitation center, etc.) where the team approach to treatment is utilized.

    • Licensure by NY State
    • Ability to travel

    What you will do:

    • Provide assessment, rehabilitation, counseling and supportive casework geared to restoring patients to their optimum level of social and health adjustment. This includes assisting patients and their families to understand, accept and follow medical recommendations, addressing legal financial issues, assessing family dynamics, coping mechanisms and potential barriers to recovery.
    • Helps patients utilize the resources of their families and the community. This may be accomplished by either referring the patients to resources or acting as an intermediary on behalf of the patients in their dealings with other health and welfare agencies.
    • Assists patients and their families with personal and environmental difficulties which predispose them toward illness or interfere with obtaining maximum benefits from medical care. These range from counseling members of the patient's family to assisting patients with admission to a nursing home.
    • Consults with the physician and other members of the health team for the purpose of assisting them to understand significant social, emotional and environmental factors related to the patient's health problems.
    • Assist in the development and revision of the Physician's (Authorized Practitioner) Plan of Treatment.
    • Attends and participates in case conferences, as indicated, to share and/or gain knowledge, especially when services are provided to special populations.
    • Participates in staff development activities.
    • Participates in Performance Improvement activities.
    • Provides quality care within guideline of agency policies procedures as well as local state and federal rules and regulations and accepted standards of care for Medical Social Workers
